ACGC Report of Noncompliance Form
A complaint is a written statement involving allegations of non-compliance with the Standards and policies and procedures of the Accreditation Council for Genetic Counseling (ACGC). The complaint may be filed by any person, including a student, a member of the general public, faculty, a government official, or representative of an organization, who has personal knowledge of possible noncompliance within an ACGC accredited genetic counseling program.

Filing a Report of Noncompliance
A complaint against an ACGC accredited program may be submitted to ACGC at any time in the year following the concern of noncompliance using this Form. Complaints should assert facts in sufficient detail to support the allegations and permit a fair investigation by ACGC and response to the complaint by the program. Relevant Standards of Accreditation and/or established policies should be referenced in the complaint.
The following information should be submitted with a complaint:
- A completed Report of Noncompliance Form
- Contact information and digital signature of the complainant
- A clear description of the alleged noncompliance
- References to relevant Standards and/or policies
- The facts and circumstances prompting the complaint must have occurred within one year of submitting the Noncompliance Form to ACGC.
- Description of the complainant’s efforts to resolve the complaint with the program, other regulatory agencies or through a legal process and the outcome of those efforts.
- Consent by the complainant to cooperating in the investigation and agreement that ACGC can disclose the identity of the complaint to the respondent program.
ACGC will consider requests to maintain the confidentiality of the complainant but may decline to process a complaint where the identity of the complainant is required to conduct a thorough investigation or for the program to submit an adequate response to the allegations.
ACGC will exercise its discretion in determining whether to investigate complaints submitted anonymously.
